Fees &
Payment Information Our fees reflect the level of care you receive as well as the
education and experience of the physicians.
All patients are asked to pay at the time of the visit unless
other arrangements have been made. We accept Master Card and Visa.
Statements are mailed monthly. Any error in the statement
should be immediately brought to the attention of the billing office. All
charges are due and payable upon receipt of the statement. Please contact our
billing department if you need to arrange a payment plan. An account over 90
days without payment will be referred to our collection agency.
We ask you to pay your co-payment at the time of service.
Our insurance department will be happy to provide you with any medical
information necessary for you to receive proper reimbursement.
Regardless of the insurance company, the patient is
responsible for any balance not covered by the insurance company.
Federal law requires physicians to collect the patient's
portion of the bill when insurance is filed. We ask that you pay any co-pay at
the time of service. |
Insurance
Many insurance
plans are accepted by Desert Valley Pediatrics, but it is important
to keep in mind that the insurance companies that we contract with
may change occasionally.
All patients are asked to check with their
insurance company to verify that Desert Valley Pediatrics is
contracted with them.
At each visit, we will ask to see your insurance card and
will file your visit with your insurance company.
Be sure to report any change in your insurance coverage,
personal status, name, address, telephone number, etc so we can maintain
current records. Make sure the physician you are seeing is in your
insurance network. |
